Introduction
NHP is the regional organization for individuals who receive primary care in the following counties:
Baca, Bent, Cheyenne, Crowley, Elbert, Kit Carson, Kiowa, Lincoln, Larimer, Logan, Morgan, Otero, Phillips, Prowers, Sedgwick, Washington, Weld, and Yuma.
NHP partners with local health care providers and community-based organizations across these counties to ensure you receive high-quality, comprehensive care. From physical and behavioral health services to connecting you with resources that support your overall well-being, NHP is here to help you get the care you need. Get Involved!
At NHP, we know that better health starts with listening. That’s why we actively invite Health First Colorado (Colorado’s Medicaid Program) members, providers, caregivers, and community partners to get involved in shaping the services and support we offer.
We host advisory committees that bring people together—from members and families to advocates, providers, and local organizations. These gatherings create space for open dialogue, shared problem-solving, and meaningful collaboration. Your experiences and ideas help us strengthen programs, improve care, and build healthier communities. Members may only attend one MEAC meeting a quarter, but may also get involved in our other Engagement opprtunities such as our Member Materiel Review Workgroup, or Program Improvement Advisory Committee. All materials available on our website are printer-friendly and can be opened and printed. If you need any document from our website in large print, Braille, other formats, or languages, or read aloud, or you need a paper copy, please contact us. We will send this to you free of charge within five (5) business days. NHP can also connect you to language services including American Sign Language or help you find a provider with ADA accommodations. Our number is 800-541-6870 or 711 (State Relay) for members with speech or hearing disabilities. These services are free.
