What is a Health Neighborhood?
A Health Neighborhood is your partner in care, working with you and your Primary Care Provider (PCP) to support your whole health. Your PCP is your medical home, and your Health Neighborhood includes the professionals and services that help meet all of your health needs.
A Health Neighborhood may include:

- Behavioral Health providers

- Long-term services and supports
