Long Term Services & Supports

Long Term Services & Supports (LTSS)

Long-Term Services and Supports  provide help with everyday activities for older adults and people with disabilities. Programs like Home and Community-Based Services (HCBS) are available based on your needs and eligibility. A local Case Management Agency (CMA) can guide you through your options and may connect you with a Regional Accountable Entity (RAE) like NHP to access additional community resources.

Case Management Agencies (CMAs)

CMAs assist individuals and families with accessing necessary services and supports to meet their needs.

Services include:

  • Eligibility determination
