Care coordination is a free benefit for Health First Colorado members.

NHP Care Coordinators are your partners in care. Navigating health care can be challenging. NHP care coordinators are here to help guide you. Our Care Coordinators are here to support you—whether you’re managing a health condition, need help with appointments, or simply have questions. You don’t have to navigate the system alone. A care coordinator can assist with:

Long Term Services & Supports

Long Term Services & Supports (LTSS)

Long-Term Services and Supports  provide help with everyday activities for older adults and people with disabilities. Programs like Home and Community-Based Services (HCBS) are available based on your needs and eligibility. A local Case Management Agency (CMA) can guide you through your options and may connect you with a Regional Accountable Entity (RAE) like NHP to access additional community resources.

Case Management Agencies (CMAs)

CMAs assist individuals and families with accessing necessary services and supports to meet their needs.

Services include:

  • Intake
  • Eligibility determination
  • Service plan development
  • Arrangement for services
  • Delivery of services
  • Service and support coordination
  • Monitoring
  • Termination and discharge from services
