Who Can Join?
We contract with providers who meet all requirements to serve as a Primary Care Provider and Medical Home in the Accountable Care Collaborative (ACC) Program, this includes:

- Be enrolled as a provider in the Health First Colorado Medicaid program
- Be one of the following:
- Certified in the Medicaid and CHP+ Medical Homes for Children Program, or
- An individual physician, advanced practice nurse focused on primary care (e.g., general practice, internal medicine, family medicine, pediatrics, geriatrics, OB/GYN), or
- A qualified Community Mental Health Center (CMHC) or HIV/infectious disease provider, or
- A Federally Qualified Health Center (FQHC) or Rural Health Clinic (RHC)
- Hold a valid license in Colorado as a MD, DO, or NP
- Serve as the dedicated source of primary care for assigned members
- Be able to deliver comprehensive, preventive, and sick care to members
- Demonstrate commitment to the Medical Home model as defined by the state
Medicaid Provider Type Codes
To begin the contracting process, your practice must be enrolled under one of the following Health First Colorado provider types:
- Physician (Code 05)
- Osteopath (Code 26)
- Family or Pediatric Nurse Practitioner (Code 41)
- Clinic or Practitioner Group (Code 16)
- Non-Physician Practitioner Group (Code 25)
- Federally Qualified Health Center – FQHC (Code 32)
- Rural Health Clinic – RHC (Code 45)
- School Health Services (Code 51)
