NHP/R2 Care Coordination Subcommittee

This monthly meeting series is mandatory for all NHP Delegated Care Coordination Entities/Practices and is intended to support our collective success. We’ll use this time to review contract requirements, share updates, provide training, and answer any questions related to delegated care coordination activities. Our goal is to ensure alignment, build consistency, and create space for collaborative problem-solving.

To help us make the most of our time together, we ask that all participants join on time, turn on their cameras whenever possible, and actively engage in discussion. If you are unable to attend a session, please notify us in advance and identify a delegate from your organization to attend in your place.
