At NHP, we partner with Primary Care Medical Provider (PCMP) sites to strengthen care delivery and support continuous improvement. Our goal is to help your practice thrive while delivering high-quality, coordinated care to Health First Colorado members.
Every PCMP site is matched with a Practice Transformation Coach. Your coach will work directly with your team to provide hands-on support and guidance using the NCQA Patient-Centered Medical Home (PCMH) model. Support is always tailored to your practice’s size, goals, and community.
