About NHP

In Colorado, Medicaid is known as Health First Colorado. Every member is part of a regional organization that manages their physical and behavioral health care. For Region 2, that organization is NHP. NHP works with you to coordinate your care—offering assessments, referrals, and support to help you access your benefits and get the services you need, when you need them. We also help you set and achieve health and wellness goals so you can live a healthier life. To make this possible, NHP partners with your primary care provider, behavioral health provider, community care teams, and local organizations to ensure you receive the right care and support.

Integrated Care

Integrated care happens when a team of Primary Care Providers (PCPs) and behavioral health providers work with Members and their families. The goal is a patient and family-centered approach to address whole person health. Providers who coordinate Members’ mental and physical health treatment help the Member have the best health outcomes.

A persons’ behavioral health connects to their physical health (and vice-versa). Integrated care has a positive impact on whole person health such as:

  • Decreased patient depression levels;
  • Improved quality of life;
  • Decreased stress;
  • Lower rates of hospitalization

Integrated health treatment may improve Member and provider satisfaction by reducing access barriers and improving communication between multiple providers.

What is a Health First Colorado Regional Organization?

Also referred to as “Regional Accountable Entities (RAE)”, “Health Plan”, or “Regional Health Plan, a Health First Regional Organization” is a Colorado regionally-oriented organization that helps you manage your Health First Colorado health benefits and connects you to the care and resources you need.

Legal Description

Northeast Health Partners is licensed by the State of Colorado, Division of Insurance, as a Limited Service Licensed Provider Network and is subject to various statutes and regulations governing the conduct of its business.

As a Regional Accountable Entity (RAE) under contract with the State, we manage physical and behavioral health services for individuals who live within the counties of Northeast Colorado.

We comply with both Federal and State regulations governing the operation of managed care health plans financed with Federal and State health care funds.

 

Colorado State Innovation Model (SIM)

The Colorado State Innovation Model (SIM) helps primary care practice sites and bidirectional health homes integrate behavioral health and primary care and test alternative payment models. The initiative, which was created by the Patient Protection and Affordable Care Act (ACA), helps progress along a continuum of integration to ensure better health outcomes at lower prices. Providers say the supports they receive through SIM will help them succeed in alternative payment models. In practical terms, SIM helps ensure that patients receive whole-person care from appropriate care providers at the right time in the right place.

Our Governance Plan describes how the Northeast Health Partners, LLC (NHP) will protect against any perceived Conflict of Interest among its governing body from influencing the Regional Accountable Entities (RAE) activities under the Medicaid contract.
