Transforming Health in Region 2
At NHP, we’re here to support your whole health. As your regional organization for Region 2, we bring together physical and behavioral health services into one coordinated plan—making it easier for you to get the care you need.
We proudly serve the following counties: Baca, Bent, Cheyenne, Crowley, Elbert, Kit Carson, Kiowa, Lincoln, Larimer, Logan, Morgan, Otero, Phillips, Prowers, Sedgwick, Washington, Weld, and Yuma.
Care Coordination
Care coordination is a free benefit for Health First Colorado Members that brings your care team together. Health First Colorado unites your entire care team to ensure you receive the right treatment.
Care coordinators assist with accessing health services and understanding your Health First Colorado Benefits. We can assist you with transportation to appointments and connecting you to community resources. Learn how NHP supports you.
